Your health is our #1 Priority and by following our policies and having an open line of communication can avoid
infections or possible dangers. If you follow our monthly pedicure regimen, it will eliminate most issues you can get
from pedicure services. Some issues that cannot be resolved such as ingrown toenails, diabetic ulcers, severe tinea pedis or any type of infection will be referred to a Physician.
If you experience any pain or discomfort while under our pedicure regimen, please call or email so we can assess the situation for proper referrals to a physician. We never know where clients are coming from and what their prior health conditions are, so by signing this waiver, you are accepting things that can happen after this visit that you cannot hold Burlington Nails & Spa liable for, as these are the risks that could be involved:
• Maintenance – Routine maintenance is required every 2-4 weeks. There are times when the product can
become “wiggly” and a small amount is normal, but if it becomes excessive it’s recommended you come to have it removed.
• Trauma- Wearing toenail enhancements can cause trauma to your natural nail, from minor to permanent
damage. Burlington Nails & Spa uses provides toenail enhancements as safe as possible. When you wear your toenail shorter, thinner and with a toenail friendly product it lessens your risk of toenail trauma.
• Longevity- We cannot guarantee the nail can stay on long if you have less than 30% of natural toenail for the product to adhere to.
• Fungal Infections – Fungal infections are very easy to get, especially if your feet are given the perfect conditions for the fungal infections to thrive. Just because you do not notice a fungal infection, does not
mean you already don’t have one. Some symptoms can either be overlooked or non-existent at first prior to showing themselves.
• Exothermic Reaction or Burning – When curing toenail enhancements, it is possible for the UV light to create an exothermic reaction. It is the process when the UV light cures the product and it “tightens up” on the nail causing a burning sensation. It’s rare, but it could happen. Just let the Medical Nail Technician know when you feel any warmth. Burning can also come from a sensitive area in your toenail or skin that makes contact
with a pedicure bur during the e-filing process. If you are sensitive in any areas on your foot, please let your pedicurist know prior to your pedicure or during.
• Ingrown toenails are a possibility for anyone with healthy or unhealthy toenails. It is important that if you experience any pain after your pedicure that you contact us right away to be referred to a Podiatrist. If you
fail to seek attention while you’re in any pain, you are at risk for infection. Burlington Nails Spa is not liable for paying for medical attention for ingrown toenails or infections coming from an ingrown toenail that could have been avoided.
By signing this form, you assume all responsibility and waive any claim for personal injury associated with our
pedicure or manicure services.
